Product Introduction
Different esterified forms of testosterone, an androgen used to treat male hypogonadism. Testosterone blend combines several esters to approximate the physiological testosterone profile and reduce peak-trough fluctuation
Therapeutic Use
Primary or secondary male hypogonadism (delayed puberty, azoospermia/infertility, post-orchiectomy, post-chemotherapy), palliative treatment of breast cancer in women (with aromatase inhibitors), gender hormone therapy; indications follow the locally approved label
Mode of Action
Ester prodrug requiring esterase hydrolysis to testosterone; enanthate/cypionate are long-acting (about 2–3 weeks), propionate short-acting (several days), undecanoate very long-acting (months); some testosterone aromatises to oestradiol, so oestrogenic symptoms must be monitored
Administration & Monitoring
Deep intramuscular injection, individualised to the patient; monitor testosterone levels, PSA, full blood count (haemoglobin), lipids and liver function; periodically review prostate and cardiovascular risk during long-term therapy; monitor androgenic symptoms and lipids in women
Important Safety Information
Prescription medicine. Abuse causes dependence, testicular atrophy, reproductive suppression and psychiatric symptoms. Prohibited in athletes. Start at a lower dose in men over 65. Intermittent dosing or switching to daily oral/transdermal delivery reduces risks such as polycythaemia
Adverse Reactions
Injection-site erythema, pain, induration; acne, oily skin, alopecia; testicular atrophy, reduced sperm count and raised PSA in men; voice deepening, hirsutism, menstrual disturbance and acne in women; oedema, weight gain, hypertension; raised haemoglobin (polycythaemia), reduced HDL; hepatic toxicity with long-term use; insomnia and mood swings
Important Risks
Polycythaemia and polycythaemia-associated thrombotic events; increased cardiovascular event risk (heart failure, myocardial infarction); prostatic enlargement/cancer risk; aggravation of sleep apnoea; dyslipidaemia; dependence and withdrawal
Contraindications
Hypersensitivity; known or suspected prostate or breast cancer (except specialist-directed palliative use in breast cancer); pregnancy (especially transdermal gels); severe cardiac, hepatic or renal impairment; uncontrolled atherosclerotic heart disease, NYHA grade ≥ III heart failure, recent myocardial infarction or stroke; paediatric and adolescent use
Special Populations
Use with caution in children/adolescents (risk of premature epiphyseal closure); reduced starting dose in men over 65 and those with cardiovascular risk; contraindicated in pregnancy; not recommended in lactation
Usage Limitations
Prescription-only; discuss dosing intervals with the prescriber/pharmacist to reduce pharmacokinetic fluctuation; prohibited for competitive athletes; rotate injection sites; do not combine with other testosterone products
Anti-Doping Testing
Testosterone is on the WADA Prohibited List S1.1 (exogenous testosterone); urine testing uses GC-C-IRMS for the testosterone/androstenediol ratio (TS/AD), with the Athlete Biological Passport for longitudinal blood data; ester prodrugs are also detected
Product Quality Testing
Identification (HPLC/MS/FTIR), assay and content uniformity, related substances, clarity and particulate matter, fill volume, sterility and bacterial endotoxin, residual solvents, stability including in-use stability; COA and lot-to-lot consistency studies
Source & Verification Note
Ester CAS numbers (58-20-8 / 315-37-7 / 57-85-2 / 5949-44-0) confirmed; testosterone blend is a mixture with no single CAS.
