Product Introduction
Recombinant human insulin-like growth factor 1. Unlike growth hormone, IGF-1 bypasses the GH receptor and directly activates the IGF-1 receptor (IGF1R / insulin-receptor-related pathway) to promote tissue growth. **It is not approved for human use in most jurisdictions** (it was historically used in some markets as an indication of recombinant GH), and is now mainly a research and sports-nutrition product
Therapeutic Use
Where approved (limited markets/historical): growth hormone insensitivity syndromes (GH resistance such as Laron syndrome), some causes of short stature, and insulin-resistance-related indications such as lipid disorders. Approval has been withdrawn in most countries; human use is largely off-label or prohibited
Mode of Action
Promotes cell proliferation, differentiation and protein synthesis through downstream IGF1R/IRS/PI3K-AKT and MAPK pathways; has insulin-like glucose-lowering activity (hypoglycaemia risk); acts upstream of the GH receptor, so it is effective in individuals with normal GH
Administration & Monitoring
Subcutaneous administration as prescribed (where an approved indication exists); monitor fasting glucose and insulin (hypoglycaemia risk), IGF-1 levels, thyroid function (may induce hypothyroidism), electrolytes, hepatic and renal function; assess tumour and bone-density risk during long-term use
Important Safety Information
**No broadly approved human indication**; non-medical use for muscle gain, fat loss or anti-ageing is prohibited and of unquantified risk. Its insulin-like action can cause severe hypoglycaemia; it may promote tumour cell growth (IGF1R signalling), so it is contraindicated in malignancy; it suppresses the endogenous GH and gonadal axes, altering thyroid and reproductive function
Adverse Reactions
Hypoglycaemia (can be severe); injection-site reactions; sodium and water retention and oedema; hypothyroidism; headache and fatigue; immunogenicity (anti-IGF-1 antibodies, rare); theoretically, promotion of tumour growth
Important Risks
Severe hypoglycaemia, especially when combined with hypoglycaemic drugs; potential tumour-promoting activity; suppression of endocrine axes (hypothyroidism, changes in gonadal function); lack of long-term safety data, particularly in off-label settings
Contraindications
Hypersensitivity; active or previous malignancy (IGF-1 signalling is implicated in tumour growth); severe hypoglycaemia; use in healthy individuals without an approved indication (muscle building and similar); pregnancy (no indication)
Special Populations
Paediatric use requires paediatric/endocrinology supervision; older adults have higher hypoglycaemia risk; contraindicated in pregnancy and lactation (no indication); medication adjustment and strict glucose monitoring when diabetes or hypoglycaemic drugs are present
Usage Limitations
**Strictly prohibited for competitive sport and muscle building**; human approval has been withdrawn in most countries, leaving research/laboratory use only; it is a controlled substance; prescription and compliant research approval required
Anti-Doping Testing
IGF-1 is on the WADA Prohibited List S2.2 (growth hormone / IGF-1); testing uses GH/IGF-1 markers (GH-200 / high IGF-1 as a flag) combined with a GH stimulation test (OGTT or high-dose arginine) for confirmation. **Use without an approved indication is necessarily a violation**
Product Quality Testing
Identification (peptide mapping/ELISA/MS), purity (SEC-HPLC), endotoxin, sterility, lyophilisate appearance and reconstitution characteristics, in-use stability and absence of aggregation (do not store after reconstitution as aggregation inactivates it), fill volume, bioactivity; COA
